NOTE:  Before completing this form, please read the ACADEMY Registration Information on "Read First page" and use a separate form for each registrant. You may want to mail or hand carry your form to the office (Grossman Commons Building – mezzanine level) as soon as possible since courses fill quickly! PLEASE PRINT LEGIBLY.

Please list below the courses that you wish to register for, in order of preference.  Remember that you can now register for up to 4 six-week courses, 2 twelve-week courses, or one twelve-week and 2 six-week courses.  Under your initial choices, please list extra choices in case any of your initial choices have already been filled. If your registration can’t be completed using your initial and/or extra choices, then you will be called to make further decisions. Additional courses may be added at no cost if any openings exist at the start of the semester.

 PRIMARY CHOICES

Assignments to classes are made according to the order in which registration forms are received. To insure your enrollment in a course, please mail or hand carry your registration form and check (Payable to A.L.L.) to the office at the address listed below as quickly as possible. The non-refundable tuition fee is $75**.There is a small but generous scholarship fund. To apply, mail the registration form and a statement explaining the need for the scholarship to the President of A.L.L. at the address shown below.
